Keep in mind that to gain your certificate, you must submit two birth verification forms within one year of the program commencement date as instructed below. You can start practicum 2 (the birth attendance experience) as soon as you start the course, although it is recommended to wait until you have completed module 1 and/or your community volunteer hours.

Attendance at 2 Births

The following must be completed for each birth:

• One copy of the birth attendance sheet to be completed by the client
• One copy to be completed by the midwife or mentoring doula.

If you attend at a hospital birth without a doula or midwife, you must obtain copies of the attendance sheet signed by both the attending physician and a labour/delivery nurse.

• A 500 word summary of each birth describing when you came into attendance, the progression of labour, what skills you used, what interventions were used, the outcome of the birth, the health of the baby, your contribution to the entire process and how long you stayed after the birth.

Please submit all of these documents to your instructor for full certification to be achieved.
